Welcome to our "Faith Formation & Religious Education" page. Here in our parish, we offer soul nourishing programs for all ages. We are blessed to have FORMED.ORG, trained catechists, programs for baptized Catholics to receive their First Holy Communion and or Confirmation, programs to formally prepare children, teens, and adults to become Catholics.  Please read the various information contained in the pull-down menus.

Youth & Children Faith Formation

A completed catechism application includes
1. Registration Application
2. Certificate Copies (Birth, Baptism, First Communion if applicable)
3. Godparent's copies of Sacrament certificates & approval letter from his/her parish
4. Diocesan waiver & Photo Release form
5. Donation to cover book cost

Please note:
If your child received past Sacraments in this parish, you would still need to provide a copy. If you need to order one, please click here. It is usually a two (2) week process.
