Confirmation Checklist Page
Both Adult and Youth learners will use the same above checklist for enrollment into catechesis and Sacramental preparation although adult learners may be subject to a different time schedule.:

- Family/Adult Learner interviews with Pastor.
- Application Form
- Birth Certificate (copy)
- Baptismal Certificate (copy)
- First Communion Certificate (copy)
- Sponsor/Godparent Certificate (Confirmation if single, Catholic Marriage Certificate if married)
- Sponsor/Godparent Letter from Pastor
- Turn in any documents if late/transferring into program. (Year 2 Students should get their Sponsor name and documents in as early this year if they have not already done it in 1st yr.; students should also have their own certificates turned in.)
- Saint Name
- Letter to the Bishop --catechist will notify learners when these are due (scroll down to yellow for an outline)
Sponsor/Godparent Certificate & Pastor Letter
Needed from Godparent/Sponsor:
- The canonical requirements (Canon Law #872-874): for godparents for Baptism and sponsors for Confirmation are the following:
- A Sponsor/Godparent must be at least sixteen years of age
- may not be one of the parents of the child/youth
- Be fully initiated into the faith (i.e. has received the sacraments of initiation: Baptism, Confirmation and Eucharist).
- Must regularly practice the faith by attending Mass and receiving the Sacraments
- Must be in good standing with the Catholic Church (i.e., not in intimate relationships outside of marriage, receives Sacraments of Confession and Eucharist regularly, etc.)
- If married, must be married in the Catholic Church or in another Church, with permission and a dispensation.
- Copy of their Confirmation Certificate (if single) or Copy of their Catholic Marriage Certificate from the church of Marriage (if married)
- Letter from their pastor stating they have permission to be a Godparent/Sponsor and are in good standing with the Catholic Church. The church seal should also be on the pastor letter.
- Fr Joey requires that Sponsor need to go through a retreat with their own parish and the pastor letter should also indicate that.
- Parents are also to attend a retreat for preparation for Confirmation preparation.
- Students will be offered a retreat either at our own parish, or if allowed, at the parish where the Confirmations will be held.
Letter to the Bishop by Confirmation Students
The letters can follow this general outline:
Greeting:
Dear Bishop Garcia,
Paragraph 1:
Introduce yourself by saying something about who you are, your family, your parish, etc.
Paragraph 2:
Ask permission to receive the Sacrament of Confirmation. Give the reasons why you'd like to receive it.
Paragraph 3:
Explain how you have prepared to receive the Sacrament of Confirmation. (You can include attending catechism classes; volunteer work for the Community, the Parish, or others; If you have done novenas or gone on pilgrimages in preparation, write about your experiences. Be truthful.)
Paragraph 4:
Talk about the difficulties that teens or others face in today's world. How can you help? How do you plan to carry out your service for God and others with the help of the Holy Spirit?

This should be a personal reflection written by the Confirmation student expressing their own personal faith journey. Your story and letter will be unique to you.
